I'm planning a trip for this fall. I've been to Cabo several times, but would like to try someplace different. Where are the hot spots in the western hemisphere?

Have you tried East Cape? Same area but about 45 min N of San Jose. Boats are much cheaper and some say fishing is better. More Tuna & Dorado June-Oct.
There are 3 good resorts own by the same guy. All have very good boats. Check out the Palmaz De Cortez web site.
Palmaz is a little more high end, nice pool. Punta is a little farther south and is pretty much just fishing.

Venezuela has a lot of fish. Fall is prime-time. I went in spring (also good but not as hot as autumn by all accounts) and saw a lot of blue marlin. Only caught one, but that's how it goes sometimes. Lots of sails and whites in the fall. Lots of other stuff to see and do in Venezuela too. Check out Los Llanos, Amazonas and Merida (Andes) if you have time. I fished with Capt Hiram Concepcion (American Capt) on the Courtesan Too and was very impressed by the his and his crew's efforts. Will be back in a flash when I get the chance. Mention I recommended him if you contact him at CaptHiC@aol.com.
Good luck. There were a lot of illegal longliners there when I went. Make a point of whinging about them!

For White Marlin no place better than Venezuela, 2nd is Punta Cana;
For sails - Guatemala hands down, 2nd is Venezuela, 3rd is Costa Rica;
For Striped Marlin - Ecuador & East Cape, Baja. I much prefer Hotel Buenavista which is upgrading their fleet. 2nd is Costa Rica;
For Atlantic Blues - Venezuela;
For Pacific Blues - Costa Rica, 2nd is Panama or Ecuador.
